Hospice of the East Bay (HEB) is a community-based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for one year after the death. Using CHO software, maintains database of bereaved family members, group members, and community referrals.
Using CHO and MS Word mail merges letters and sends the initial bereavement letter within 10 days of receipt of the bereavement referral to the family member. Also produces and sends the 6-month letters, one year anniversary letters, and invitations to support groups/classes/meetings in a timely manner.
Creates and/or builds databases on Access or Excel when necessary for specific bereavement mailings.
Using Microsoft Word or Publisher creates flyers, invitations, written programs, rosters, attendance sheets, and confirmation letters.
Maintains the bereavement calendar of support groups/classes/events.
Reserves conference rooms for bereavement groups/classes/meetings.
Sets up and maintains files for all support groups.
Develops and maintains bereavement support group schedule and prepares material and/or handbooks for ongoing groups and classes.
Assists in training new facilitator’s with support group set up and closing up office.
Prepares all promotional informational packets for special events such as workshops and educational meetings.
Files communications and volunteer progress notes in a timely manner.
Provides assistance to Bereavement Manager for the Annual Evening of Remembrance, monthly staff memorial service and bereavement volunteer meetings, to include: data entry, preparing lists of names to be read and room set up/clean up.
Assists in the Bereavement Volunteer Training process by setting up files, developing and maintaining volunteer rosters, copying training materials, assembling manuals, creating signage, name badges and attendance list. Helps set up training room and refreshments.
Prepares for the monthly bereavement volunteer meetings, including the purchase of refreshment, preparing and setting up the room and cleaning up after the meeting.
Sets up and maintains files for bereavement volunteer’s and mails out bereavement volunteer assignments and updates volunteer client roster.
Gathers necessary supplies, performs set up and clean up and procures and assembles refreshments for the Footsteps program.
Handles incoming calls and updates phone system greetings.
Uploads monthly data to DEYTA site for FEBS survey by the 15th of each month, prints reports monthly and monitoring call back requests.
Weekly data entry into CHO from Bereavement Referral Forms generated in EMR so records are created before counselors make three week calls to family members.
Tracks statistics for measuring department and agency goals.
Prepares notes from department meetings for distribution to department staff in a timely manner.
Records patient deaths daily in green binder.
Distributes bereavement calendar to agency via email and in clinical staff boxes as the calendar changes.
Forwards staff schedules to front desk staff on a weekly basis.
On a monthly basis, purges written files and scans onto I: drive.
Files face sheets into appropriate team binders.

Position Requirements

Excellent interpersonal and organizational skills. Proficiency with Microsoft Office 2003 (Word, Access, and Excel). Familiarity with networked computer systems. Ability to create databases, familiarity with Publisher and PowerPoint a plus. Expert ability in mail merging letters. Ability to type at least 50wpm. Ability to prioritize and understand written and oral communications. Must be highly organized, able to work independently and multi task on a daily basis. Have knowledge of basic office equipment. Ability to speak with patience and compassion to callers over the telephone. Ability to stay calm and present when a caller is sad or tearful. Ability to work with and respect volunteers. Ability to represent the agency to the public with a professional image.

Experience & Education

High school diploma or some college preferred.
Advanced coursework or proven ability to work with listed computer software required.
Previous experience working in a health care or non-profit setting a plus.
