We are looking for a Social Services Assistant to join our team!!
Woodland Skilled Nursing is a 5-Star rated facility located in Woodland, California and is nestled in the heart of Yolo County. Located in the building that served as the original hospital for Yolo County, Woodland Skilled Nursing shares in a long and rich history of taking care of the members of our community. We deliver a high standard level of care for our residents, and strive to maintain a warm, safe, and welcoming environment for all.
Purpose of Your Job Position
The primary purpose of your job is to assist in the implementation and operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as directed by the Director of Social Services and/_or the Administrator, assuring that the medically related emotional and social needs of the resident are met/_maintained on an individual basis.
Administrative Functions
– Assist the Director of Social Services in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility
– Keep abreast of current federal and state regulations, as well as professional standards
– Participate in discharge planning, development and implementation of social care plans and resident assessments. Interview resident/families to obtain social history
– Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director as required
– Involve the resident/family in planning social service programs when possible
– Coordinate transportation to follow-up appointments, for discharges, or to other facilities when necessary
– Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident
– Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident
– In the absence of the Director assume the authority, responsibility, and accountability of directing the social services department
– Maintain a quality working relationship with the medical profession and other health related facilities and organizations
– Coordinate social service activities with other departments as necessary,
– Work with the facility’s consultants as necessary and implement recommended changes as required
– Make written and oral reports/recommendations to the Director concerning the operation of the social service department
– Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required
– Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other resident care team members
– Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident
– Assist the Infection Control Coordinator in identifying, evaluating, and classifying routine and job related social functions to ensure that tasks involving potential exposure to blood/body fluids are properly identified and recorded
– Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken
– Participate in facility surveys (inspections) made by authorized government agencies
– Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident’s response to the service
Care Plan and Assessment Functions
– Assist in developing preliminary and comprehensive assessments of the social service needs of each resident
– Assist in developing a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified
– Encourage the resident family to participate in the development and review of his/her plan of care
– Assist in the scheduling of care plans and assessments to be presented and discussed at each committee meeting
– Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care
Resident Rights
– Maintain the confidentiality of all resident care information
– Knock before entering a resident’s room
– Review complaints and gri_evances made by the resident and make a written/oral report to the Director indicating what action(s) were taken to resolve the complaint or grievance. Follow facility’s established procedures, maintain a written record of the resident’s complaints and/_or grievances that indicates the action taken to resolve the complaint and the current status of the complaint
– Provide residents with information concerning resident rights, living wills, etc. Explain as necessary
– Participate in resident group council meetings as requested and provide support services to such council
Working Conditions
– Works in office areas as well as throughout the facility
– Moves intermittently during working hours, Is subject to frequent interruptions.
– Is involved with residents, personnel, visitors, government agencies/personnel, etc. under all conditions and circumstances
– Is subject to hostile and emotionally upset residents, family members, etc.
– Communicates with the medical staff, nursing service, and other department supervisors
– Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary
– Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.)
– Attends and participates in continuing educational programs
– Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants
– Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses
– Maintains a liaison with other department supervisors to adequately plan for social services/activities
– May be subject to the handling of and exposure to hazardous chemicals
Education
– Must possess, as a minimum, a High School Diploma, or equivalent
Experience
– Experience in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology preferred
Specific Requirements
– Must be able to read, write, speak, and understand the English language
– Must possess the ability to make independent decisions when circumstances warrant such action
– Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
– Must possess leadership ability and willingness to work harmoniously with other personnel
– Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning
– Must be able to relate information concerning a resident’s condition
– Must not pose a direct threat to the health or safety of other individuals in the workplace
Physical and Sensory Requirements (With or without the Use of Mechanical Devices)
– Must be able to move intermittently throughout the work day
– Must be able to cope with the mental and emotional stress of the position
– Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
– Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies
– Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination
– Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility
– Must be able to lift up to 20 pounds. Constantly carrying charts. Frequently pushes carts or transports residents
– May be necessary to assist in the evacuation of residents during emergency situations
